Release of Personal and Medical Information
By accepting membership in Lifeteam AlertTM, a member authorizes
Lifeteam Alert to release all medical and other confidential
information about its members, in emergencies, and to other health
care personnel that our members designate. If a member chooses to
terminate membership, we ask that the member notify in writing and
return all emblem(s) to: Lifeteam Alert, P.O. Box 349, West
Plains, MO 65775.
Lifeteam Alert relies upon the accuracy of the information members
supply. A member therefore agrees to defend, indemnify and hold
Lifeteam Alert (including its employees, officers, directors and
agents) harmless from any claim or lawsuit brought by member or
others for injury, death, loss or damages arising in whole or in
part out of Member's provision of incomplete or inaccurate
information to Lifeteam Alert.
Privacy of Medical Information
Lifeteam Alert ensures that any and all personal and medical data
are provided voluntarily by its participants. Lifeteam Alert
provides its participants with full disclosure regarding uses of
their medical and personal information. Lifeteam Alert obtains prior
approval for all storage and transfer – electronic or otherwise – of
participant’s information. Lifeteam Alert and its parent company Air
Evac Lifeteam have taken steps to protect participant information
stored on its database and ensures that the “information loop”
between its participants, Lifeteam Alert and emergency medical
personnel remains closed, thereby avoiding any breech of a
participants confidence and security. Program participants and the
emergency medical personnel who serve them can be assured that
personal and medical information stored and maintained by Lifeteam
Alert remains secure, confidential, and used only for its
authorized, intended purpose.
Information Sharing and Disclosure
Lifeteam Alert policies provide for the safeguarding of
participant’s information and prohibit its distribution (selling,
loaning or renting) to any party except for the purposes for which a
participant has subscribed. Lifeteam Alert will release a
participant’s personal, identifiable information only when we have
been granted permission to share the information for the use it is
intended which might include relaying information to emergency
personnel in medical emergencies to assist with the speed of
treatment of our members.
Lifeteam Alert depends on the information provided by its program
participants, or the participant’s parent(s) or legal guardian(s),
and does not ask for additional information other than that
necessary to provide emergency and health care information and to
contact the participant regarding renewal of their subscription.
